PROGRAM MANAGER Community Supported Living - Remote Opportunity We currently seek a business savvy human services professional to develop, implement and manage a business plan for the Wisconsin market that supports area goals and outcomes. Primary focuses will be managing a team of direct care staff; meeting with community resource partners to promote a positive community presence; working directly with funding agency personnel and creating outcome-based care plans.
Research and develop program objectives
Develop and maintain positive working relationships with agency personnel
Be an active member of the local community
Promote awareness of GT Independence and the services offered
Foster a positive work environment that attracts, retains, and motivates top quality employees
Create opportunities for staff training
Coach and counsel staff regarding the performance expectations of their position
Evaluate job performance formally on an annual basis
Provide ongoing communication, support, and education to clients, families, and caregivers
Provide continual evaluation of processes and procedures; responsible for suggesting methods to improve area operations, efficiency and service to both internal and external customers
Meet with Members to determine individual needs, goals and rate to ensure desired outcomes are met as set by the Agency
Works as a member/leader of special or ongoing projects that are important to area/process improvement
Bachelor's degree in related Human Service Field
2 years previous business development experience
PC proficiency including MS Office
Strong writing, communication and organization skills
Ability to understand and work well with a variety of financial and human service disciplines
Previous experience with housing counseling, creating care plans and an outcome based program is preferred.
