Comprehensively plans for case management of targeted patient populations within high-risk categories and caseload.Participates in departmental and system performance improvement initiatives.Promotes CSJH Culture of Excellence and quality service through team work, respectful communication, and maintenance of a professional environment.
Minimum Qualifications Required
Education:. Graduate of professional school of nursing required.Bachelor's Degree in Nursing preferred.
Experience:Minimum ofthree (3) years to five (5) years' experience in clinical healthcare setting with direct patient care responsibilities reflecting planning, coordination, delivery of needed services such as education, discharge planning and utilization management required.
Licensure, certification, and/or registration:Current licensure in Virginia as Registered Nurse required.Case Management Certification preferred.
Other Minimum Qualifications:. Strong oral and written communication skills as well as effective interpersonal skills.Must be effective at teaching individuals from all socio-economic and cultural backgrounds.Must have ability to work without close supervision and demonstrate effective organizational abilities.
